ADMISSION CRITERIA
Individuals appropriate for adult day care are those with physical, psychological, or cognitive impairments who require assistance and supervision throughout the day.
Eligible participants are:

Individuals who require assistance with activities of daily living and instrumental activities of daily living.

Individuals with physical impairments that require monitoring and supervision on a regular basis.

Individuals with memory loss and/or cognitive impairment which limits one’s ability to function independently.

Individuals with a developmental disability.

Individuals who require assistance in overcoming social isolation associated with functional limitations or disabilities.

Individuals whose family/caregiver needs respite.
-
Community Care Services clients that meet the level of care and financial eligibility for Medicaid.
Individuals who are inappropriate for adult day care are:

Individuals who are bedfast or do not have the strength and stamina to attend the program and participate in activities.

Individuals in an infectious stage of a communicable disease.
-
Individuals with emotional or behavioral disorders who are harmful to themselves or others, or who are disruptive in a group setting.

Individuals who are active substance abusers.

Individuals whose needs exceed center resources.
-
Individuals who would be more appropriate served in another setting.
